Overview

Aesthetic medicine is often approached as a series of isolated treatments.
In reality, facial ageing is a multi-layered, structural process involving changes in bone, soft tissue, muscle activity, and skin quality.

Effective treatment requires more than product selection.
It requires diagnosis, planning, and restraint.

A Structural Approach to the Face

The face is assessed as an integrated system across three key levels:

Structural Support (Bone & Deep Architecture)

Loss of projection and support alters facial balance, particularly in the midface, chin, and jawline.

Soft Tissue Position (Fat Compartments & Ligaments)

Descent and redistribution of fat contribute to:

  • jowl formation
  • deepening folds
  • loss of definition

Skin Quality (Dermal Integrity)

Changes in collagen, elastin, and hydration affect:

  • texture
  • elasticity
  • overall skin health

Treating only one layer often results in incomplete or unnatural outcomes.

Diagnosis Before Treatment

All treatment planning begins with:

  • detailed facial assessment
  • evaluation of movement and expression
  • analysis of ageing pattern

This allows identification of the underlying cause, rather than treating visible symptoms alone.

Individualised Treatment Planning

No two patients are treated the same.

Treatment plans are tailored based on:

  • anatomy
  • degree and pattern of ageing
  • skin quality
  • individual goals

Recommendations may include:

  • non-surgical treatments
  • staged interventions
  • or, where appropriate, surgical options
Precision Over Volume

Modern aesthetic treatment is not defined by quantity.

Overuse of product can:

  • distort facial proportions
  • create heaviness
  • compromise long-term outcomes

A conservative, anatomy-led approach prioritises:

  • accurate placement
  • appropriate indication
  • long-term balance
Staged Treatment Strategy

In many cases, treatment is performed in stages.

This allows for:

  • controlled, natural progression
  • assessment of response
  • avoidance of over-treatment

The goal is refinement over time, not immediate transformation.

Integration with Surgery

Non-surgical treatments are considered within the broader context of facial plastic surgery.

They may:

  • enhance surgical outcomes
  • support recovery and skin quality
  • delay the need for surgery in selected patients

However, they do not replace surgical intervention where structural correction is required.

A Measured Approach

Not every concern requires treatment.

In some cases, the most appropriate recommendation may be:

  • to delay intervention
  • to stage treatment
  • or not to proceed

Clinical judgement is central to decision-making.
