Surgical Procedures

Refined surgery of the face, eyes, nose and neck
"Where precision restores what time has altered."
Facial plastic surgery, when practised at a high level, is not the arbitrary alteration of features. It is the restoration of structure, proportion, contour and balance across the face and neck.

A significant part of his practice is devoted to facelift and neck lift surgery, together with procedures that restore youthful definition to the upper face and eye area, including blepharoplasty, brow lift, facial fat transfer and lip lift procedures. He also performs rhinoplasty, both for refinement of nasal form and, where appropriate, improvement of nasal breathing.
Begin with Consultation
Facial surgery is always planned individually. During consultation, a detailed assessment of facial anatomy, tissue position and structural ageing is performed before any recommendation is made.

The Foundation
Anatomy first
A deep understanding of the structural anatomy of the face, neck and airway informs every stage of surgical planning. This begins in the consultation phase and runs from diagnosis and patient selection through to operative technique and recovery.
The range
Beyond just surgery
A carefully selected range of non-surgical treatments is offered where these complement surgery, support skin quality, or provide subtle enhancement when surgery is not indicated.
the philosophy
Restoring harmony
The guiding principle is always the same: the aim is not to impose change, but to restore harmony. Results should look considered rather than obvious, preserving the individual character of the face rather than working against it.
Facial Rejuvenation

What Ageing Actually Changes

Time alters the face in ways that are both visible and structural.
What begins in youth as a naturally lifted, gently tapered facial form gradually broadens and descends. The cheeks lose projection, the lower face becomes heavier, the jawline softens, and the neck develops laxity. In parallel, the skin loses elasticity, the ligaments that once held the face in place begin to relax, and the deeper support layers of the face descend under the influence of gravity and biological change.

At the centre of this process lies the SMAS (the superficial musculo-aponeurotic system) a fibromuscular layer beneath the skin and superficial fat that provides structural support to the cheeks, jawline and neck. As the SMAS loosens with age, deeper tissues shift downward, contributing to jowls, flattening of the midface, loss of jawline definition and neck laxity.

For this reason, contemporary facial rejuvenation surgery addresses the deeper anatomical layers of ageing rather than simply tightening the skin. This is the basis of modern facelift surgery, and the reason refined results now look more natural, balanced and enduring than the overly tightened outcomes associated with earlier techniques.
The role of surgery

Surgical Procedures

Each procedure is selected individually, based on anatomy and the pattern of ageing and not as an isolated correction.
01
Face Lift & Neck Lift
A major part of Dr Lew-Gor’s practice is devoted to facelift and neck lift surgery. These procedures are designed to restore support to the cheeks, jawline and neck by repositioning the deeper anatomical layers that descend with age. In most patients, the face and neck are treated together, as ageing rarely affects one without the other.
02
Eyelid & Brow Surgery
The eyes and upper face strongly influence whether a face appears rested, open or heavy. Blepharoplasty and brow lift procedures are planned with close attention to the relationship between brow, eyelid and forehead, ensuring that refinement of the upper face remains natural and structurally appropriate.
03
Rhinoplasty
Rhinoplasty may be performed for aesthetic refinement, functional breathing concerns, or both. As with all facial surgery, the nose is considered in relation to the rest of the face rather than as an isolated feature.
04
Structural Refinement Procedures
Procedures such as lip lift, chin augmentation and facial fat transfer may be used to improve proportion, restore support and refine profile harmony. These are often subtle interventions, but in the correct patient they can have a powerful effect on facial balance.
Surgical Philosophy

Restoration of Structure & Preservation of Identity

The finest facial surgery should never appear surgical. For Dr Sim Lew-Gor, rejuvenation is not about transformation, exaggeration or trend. It is about restoring the structural relationships that ageing gradually alters, while preserving expression, proportion and recognisable identity.
01 – Principle

Anatomy Before Technique

The visible signs of ageing are rarely caused by the skin alone. Facial ageing occurs across multiple anatomical layers, including: skin, fat, retaining ligaments, muscle, and bone. Treatment must address the correct layer, not simply what is visible at the surface.
02 – Principle

Structural Repositioning

Modern facial surgery focuses not on skin tension but on structural repositioning. Procedures are planned according to anatomy. The technique must serve the patient – an approach that produces results which appear natural, balanced and enduring rather than operated.
03 – Principle

The Whole Face

No procedure is planned in isolation. Each structure contributes to the overall impression of the face. The objective is not to change one feature but to understand how every element (brow, eyelid, midface, jawline, neck) relates to the whole, and to act accordingly.
"The best results are not dramatic in a theatrical sense. The face appears more rested, more balanced, more structurally coherent. Expression remains intact. Identity remains intact."
What returns is not a new face but a more resolved version of the existing one. Facial rejuvenation surgery works with what the face has lost – restoring structure and proportion rather than imposing a new result.

This is the distinction between surgery that looks surgical and surgery that simply looks right.
Surgery & Non-Surgical Treatment

Not opposing approaches, but different tools

Non-Surgical

Non-Surgical Treatments

Non-surgical treatments can be effective where the concern is skin quality, mild volume change, dynamic lines or early ageing. Surgery becomes more relevant when the dominant issue is structural — tissue descent, jowling, neck laxity, eyelid heaviness, or imbalance of facial support.
Skin quality and dermal regeneration
Mild volumetric change
Dynamic expression lines
Early-stage ageing
Supporting and prolonging surgical results
Surgical

Surgery Procedures

Surgery becomes the appropriate response when the dominant issue is structural. Non-surgical treatment cannot reposition tissues that have descended and attempting to compensate with volume often leads to diminishing and unnatural results.

Structural tissue descent
Jowling and jawline loss
Neck laxity
Eyelid heaviness
Imbalance of facial support
"In many patients, the most sophisticated treatment plan includes both – surgery to restore deeper structure, and non-surgical treatments to support skin quality and longevity."
The important distinction is not whether a treatment is surgical or non-surgical, but whether it is anatomically appropriate. This is assessed honestly at every consultation.
The guiding principle
The aim is not to impose change, but to restore harmony while preserving the individual character of the face using whichever approach anatomy requires.
natural, proportionate, & enduring

Results & Expectations

The aim of facial plastic surgery is not to create a different face. It is to restore definition, support and harmony in a way that remains recognisably the patient’s own.

The best results are not dramatic in a theatrical sense. They are quieter than that. The face appears more rested, more balanced, more structurally coherent. Expression remains intact. Identity remains intact. What returns is not a new face, but a more resolved version of the existing one.
what to do Before Aesthetic Surgical Treatment

Careful Assessment Before Surgery

"The purpose of consultation is to determine the right plan."
Facial surgery is always planned individually. During consultation, Dr. Sim Lew-Gor performs a detailed assessment of facial anatomy, skin quality, tissue position and the pattern of structural ageing affecting the face and neck.

In some cases this leads to surgery. In others, a less extensive approach or a non-surgical alternative may be more appropriate.
What consultation involves
Anatomy
Detailed assessment of facial anatomy, including skeletal structure, soft tissue position, and the pattern of ageing present.
Skin Quality
Assessment of skin elasticity, thickness and condition, factors that influence both surgical planning and likely outcomes.
Tissue Position
Evaluation of where descent has occurred and which anatomical layer is most responsible for the visible change.
Options
A discussion of appropriate treatment options, including the possibility that surgery may not be the most appropriate intervention.
Realistic Outcomes
An honest account of what is achievable, what recovery involves, and what the limits of surgical correction are for that individual.
